Records Requests – Unified Fire Authority adheres to the Government Records Access and Management Act (GRAMA). GRAMA is the records law for the state of Utah. GRAMA defines what a record is and establishes the criteria for accessing government records.

Request to your Medical Records– Unified Fire Authority complies with the Health Insurance Portability and Accountability Act (HIPAA), including the breach notification rules of the Health Information Technology for Economic and Clinical Health (HITECH Act). UFA has in place policies and procedures for using and disclosing patient information, safeguarding patient information, and for providing patient access.
